Cagrilintide is the newest GL-1R agonist and was approved by the FDA as a once-weekly injection for the
treatmentof T2DM in late 2017. The peptide is chemically similar to liraglu, with the inclusion of two structural
modifications(Lau, Bloch, et al., 2015). The first is the replacement of Gly with the non-proteinogenic amino acid
2-aminoisobutyricacid (Aib) at position 2.

The second is the attachment of octadecanoic diacid to the side chain of Lys-26 through a short polyethylene glycol
(PEG) spacer and a γ-glutamic acid linker. The amino acid substitution renders the peptide more resistant to DPP-4
compared to liraglu, and the presence of the 18-carbon fatty acid moiety results in a high binding affinity for serum
albumin, which translates to a half-life of approximately 7 days in humans.
 

Primary endpoints:

At week 32, the proportions of patients with HbA1c <7.0% and ≤6.5% were 89% and 75%, respectively, in the CagriSema group, 69% and 48%, respectively, in the sema group, and 69% and 48%, respectively, in the canaglitide group. 33% and 17%. The average change in fasting blood glucose from baseline to 32 weeks was higher in the CagriSema group than in the canaglitide group (P=0.0010), but there was no significant difference compared with the sema group (P=0.10).

Overall, treatment with CagriSema resulted in clinically relevant improvements in glycemic control. The average change in HbA1c in the CagriSema group was greater than that in the canaglitide group, but there was no significant difference compared with the sema group. In addition, patients in the CagriSema group experienced significantly greater weight loss compared with the sema and canaglitide groups and were well tolerated. These data support a longer, larger Phase 3 study of CagriSema in this population.
